We at Enhance Eye Care understand that our patients want to have their office visit be as smooth as possible.  To help facilitate this process, we have provided our patient forms online.  You may print and complete the following forms, then bring them to your appointment.  You may also email the completed form to us at info@enhanceeyecare.com.

Things to remember for you appointment:

* Parents must accompany children under the age of 18 to sign the necessary paperwork

* Bring the following items to your visit:

• Photo ID
• Medical insurance card and vision insurance card (if applicable)
• All your eyeglasses, including regular sunglasses
• Contact Lens wearer: please bring your most recent prescription and/or contact lens boxes and solutions
• Bring all eye medications.
